(1) If you wish to submit receipts for reimbursement, contact your insurance carrier for current coverage information and reimbursement guidelines. You may also need a referral from your doctor. Although SLSSV does not accept insurance, you may need some sort of authorization to submit receipts for reimbursement from a health care account.

(2) Contact Speech-Language Services of Silicon Valley for further information and/or scheduling. 

(3) Make copies of any previous pertinent reports or notes (i.e. neurology, speech-language pathology, Individualized Education Program (IEP), Individualized Family Service Plan (IFSP), etc.).

(4) Complete the forms that SLSSV has e-mailed or faxed to you. If you do not have access to a printer, contact us to have these forms mailed to you.
